JOB DESCRIPTION: SUMMARY OF POSITION Engages with individuals and/or familiesexperiencing a recent crisis and assists with navigation of the mental health system and community resources. Travel across the statewide service area is required. Fosters relationships with existing external partners by participating in community meetings, committees, educational forums, etc. Actively works to establish new relationships with external resources within the community. May participate in logistical planning, overcoming obstacles, addressing stigma and providing support to clients. Performs a continuing review of the client's needs and functioning, maintenance of appropriate documentation, links clients to appropriate services and advocates on their behalf. Provides support in an active treatment environment, client-centered consultation. ESSENTIAL DUTIES & RESPONSIBILITIES

QUALIFICATIONS Education Level Bachelor's degree required. Years of Experience Experience navigating multiple youth and/or adult mental health service systems preferred. Certification/Licensure Must meet Mental Health Professional status. CPR Certification preferred.
