Overview
Performs all functions of pharmaceutical care required of an ambulatory pharmacist. Responsible for providing medication therapy management services to assure that drug therapy is safe, effective, efficient and cost effective. Assists patients, providers and partners with chronic disease state management and education as part of an interdisciplinary team. Completes patient assessments and follow-up necessary to comply with specialty pharmacy accreditation standards. Collects and analyzes data for process improvement projects. Responsible for assisting or leading projects involving specialty pharmacy which may include sharing information and improving processes with retail pharmacy partners. Will be required to take after hours call on a rotation basis. Performs other duties as assigned or as necessity dictates. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES

EDUCATION AND EXPERIENCE Minimum - Graduate of an accredited Pharmacy program Preferred - Completion of an ASHP Accredited Residency training program CERTIFICATION/LICENSURE Pharmacist license (or eligible for licensure) in the State of Illinois and/or Wisconsin based on practice setting Pharmacist license in both Wisconsin and Illinois may be required Approved CPR or BLS certification required within 90 days ADDITIONAL REQUIREMENTS Passing the Drivers License Check and or Credit Check (for those positions requiring). Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check. Must be able to follow written oral instructions. OTHER SKILLS AND ABILITIES Knowledge of retail pharmacy operations, prior authorization processes and patient copay or assistance programs Computer experience, including internet and email. Strong accuracy skills Excellent interpretive analytical skills for pharmaceutical calculations for drug dosage, IV rates, etc. WORK ENVIRONMENT The noise level in the work environment is usually moderate. LEVEL OF SUPERVISION Work requires a moderate level of independent judgment along with good interpretative and interpersonal skills with justification and accountability to Manager, and Director. SUPERVISES This position supervises technicians. This position is involved in the recruitment and orientation process and provides performance feedback and input for performance evaluation purposes. May implement coaching, counseling, and corrective action when needed.
