Department:- Case management PART Time (20 hours per week)- Day Shift- 8:00am-4:30 pm- minimum of 2 shifts per 6 weeks with 1 shift being on a weekend- no holidays Henry Ford Providence- Novi GENERAL SUMMARY Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge. Create plan for care across the continuum, integrating patient/family preferences and values. Monitor patient care through assessments, evaluations and/or patient records. Advocate for resources and removal of barriers. Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan. Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care EDUCATION/EXPERIENCE REQUIRED Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire. BSN Preferred CERTIFICATION/LICENCES REQUIRED Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association accepted. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Case Manager credentialed from the American Case Management Association (ACMA) preferred.
