Responsibilities

Case Manager

Full-Time (Monday - Friday): $21.00 - $24.00 per hour (based on experience)

Benchmark Behavioral Health System Hawaii, a community based residential facility, is seeking a full-time Case Manager. Benchmark's Case Manager is a part of the Clinical team, works closely with and is supervised by the Clinical Director. Our Case Manager coordinates all activities regarding client admission, progress, discharge, and the client's relationship with the facility, guardians, treatment team members, referring agencies, and community resources. Benchmark's Case Manager also assists the Unit Manager with the planning and scheduling of client activities off campus.

Qualifications

Requirements:

Bachelor's degree in social work, Psychology, or a related field perferred. A minimum of one year of experience in case management or related work. One year of experience in mental health services required. Must have good computer skills, be capable of tracking data on spread sheets and know how to coordinate zoom meetings with other agencies, families and individuals. Must be able to demonstrate effective communication skills and attention to detail.
