Group Health Cooperative of South Central Wisconsin. BETTER TOGETHER

1.0 FTE/ 40 Hours per Week. Will consider hiring down to .8 FTE/32 Hours per Week.

Position will split time between our Sauk Trails Clinic and East Clinic.

The Diabetes Nurse Educator provides diabetes-related care and education to members with diabetes and coordinates care with practitioner, nursing and related health education staff. The incumbent incorporates clinical, educational and administrative components of a diabetes education program. Work is performed under the general direction, guidance, and supervision of the Clinical Health Education Manager.

JOB SPECIFIC QUALIFICATIONS



  1. State licensure as a Registered Nurse required.
  2. Certification as a Diabetes Educator or working towards certification required. The incumbent must obtain certification within a reasonable time period.
  3. Minimum of two (2) years experience managing the care of a diabetic population preferred.
  4. Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required. Employee must obtain certification within four (4) months of employment.
  5. Ability to travel to various work sites. Employee is expected to provide personal transportation. Possession of a valid Wisconsin driver license if the employee is operating a motor vehicle.
  6. Knowledge of basic counseling skills and techniques required.
  7. Ability to be culturally sensitive and work with a diverse population.
  8. Ability to perform patient assessment and design individualized patient action plans required.
  9. Ability to coordinate patient care and follow-up required.
  10. Ability to maintain patient confidentiality required. Knowledge of HIPAA requirements.
  11. Ability to read, develop, write, explain and critique a variety of creative written patient education materials required.
  12. Knowledge of data entry software programs preferred.
  13. Knowledge of and ability to use computers. Familiarity with Microsoft Office programs. Ability to learn and use electronic medical record systems such as EPIC.
  14. Ability to work with clinic and administrative staff required. Ability to be a team player in a team oriented environment required. Ability to interact with staff at all levels required.
  15. Excellent communication and interpersonal skills required.
  16. Professional manner necessary to deal courteously and effectively with patients and the public required.
  17. Ability to speak and listen in person and on the phone required.
  18. Ability to lift, carry and push a maximum of 10 pounds required.
  19. Ability to see at near, mid and far range required.
  20. Ability to perform fingering and handling with both hands required.
  21. Ability to sit 90% of the time with intermittent bending and standing required.
  22. Ability to work indoors required.
  23. Ability to work flexible hours including some early evening hours, as required.
