Responsibilities

Sierra Vista Hospital is seeking Step-down Navigator for the Outpatient Department. If you're passionate about working in the behavioral health setting and have the desire to join a team of healthcare professionals dedicated to serving our patients, we want to hear from you!

The Step-Down Navigator will be responsible for managing the workflow of the step-down process. They will ensure eligible patients are identified and engaged in OUTPATIENT discussion as a next step in treatment prior to inpatient discharge. The Step-Down Navigator will be responsible for data collection, communication and report generation to the involved departments and facility CEO.

The Step-Down Navigator is responsible for the entire workflow of the step-down process. They are responsible for meeting with every eligible patient to discuss Outpatient services, methods of transportation, virtual programming options and potential start-dates. This engagement should begin once the treatment plan has been completed by the Social Worker.


Qualifications

Education: Masters degree required

Experience: A minimum of two (2) years experience in patient engagement in a psychiatric treatment environment

Additional Requirements: CPR Certification and successful completion of the hospital's designated behavioral and physical management training program, including appropriate use of seclusion and restraint required before participating in the procedures. May be required to work overtime and flexible hours or shifts (a.m., p.m. or NOC)
