OVERVIEW:

The Casework Counselor will offer comprehensive support services such as case management in regards to Intake Process and linkage to Mental Health Services, ongoing compliance, Linkage to Housing and/or Program upon Discharge as well as AOD counseling as needed.

DUTIES AND RESPONSIBILITIES:



  • Provide case management services to participants
  • Maintain accurate and timely records in WellSky and communication with the Clinical Team
  • Adhere to organizational and licensing standards, informing participants about confidentiality and obtaining ROIs
  • Assure all linkage is in place with Medical Care Providers for each participant
  • Oversee medication compliance, prescription refills, and communication with providers when behavioral changes are observed
  • AOD Counseling as Needed
  • Teach or Coordinate Life-Skills Classes as it pertains to self-care and transitioning back into society
  • Lead Group(s)/Class as needed
  • Attend and participate in case reviews, staff meetings, and pieces of training
  • Coordinate and Link participants to a continuum of care upon exit to ensure a smooth and successful transition
  • Other related duties as required


EDUCATION AND EXPERIENCE:



  • High school diploma or equivalent (required)
  • Experience in case management (1 year required)
  • AOD Credentials Registered or Certified (required)
  • Experience with Participant Documentation and Electronic Record Keeping
  • First Aid and CPR certification (within first fourteen days of employment - company provided)
  • Tuberculosis clearance (within first seven days of employment - company provided)
  • Experience and ability to work successfully with issues of substance abuse, mental illness, and other potential barriers to recovery and self-sufficiency

PHYSICAL REQUIREMENTS:



  • Ability to lift up to 25 lbs. (Usually file boxes)
