About the Practice Greenville Family Medicine & Residency is a busy fast paced clinic focused on utilizing teamwork and communication to provide compassionate care to the patients of our community. In our practice we have six family medicine providers, nine resident providers, and offer additional on sight resources for patients such as a clinical pharmacist, care manager, social worker and population health nurse. Our clinic offers a welcoming environment that thrives on strong collaboration to coordinate and optimize care for our patients. About Outpatient Clinics Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. Scope of work Serves as a population health subject mater expert in the practice. Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations and directs patients to the right care team member at the right time. Demonstrates expertise, current knowledge in nursing care and management of a sub population of patients of varying complexity and seeks to improve patient, family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment.
