RN - Case Manager

Spectrum Healthcare Resources has an Exciting Potential Opportunity for a RN Case Manager supporting The Veterans Administration Office of Community Care in Glendale, CO!

U.S. Citizenship and residency in the U.S.
• Nursing degree from an accredited school (ACEN or CCNE).
• Active, unrestricted RN license in any U.S. state or territory.
• Minimum 2 years of case management experience in the last 3 years.
• Current BLS certification and strong computer skills.
• Experience with utilization review and care coordination.

• Maintain patient confidentiality and ensure a safe care environment following VA and Joint Commission guidelines.
• Review and process consult requests, ensuring completeness and appropriateness of care.
• Coordinate community care by reviewing medical records and communicating with providers, veterans, and staff.
• Attend team huddles, staff meetings, and complete clinical alerts in CPRS within seven days.
• Provide patient-centered care, assess needs, and develop/update care plans.
• Report changes in patient conditions and notify physicians as needed.
• Ensure compliance with HIPAA, PPE, hand hygiene, and VA policies.
• Respond to emergency situations, assist in admissions/discharges, and deliver lab specimens.
