Saint Therese - St. Mary of the Woods is looking for an experienced MDS/RAI Coordinator (RN/LPN) to join our dedicated team! St. Mary of the Woods was built with a deep Catholic foundation, and it has an impeccable reputation for quality care in the marketplace. Built in 2005, it has 179 total units with 81 independent living apartments, 48 assisted living apartments and 50 rooms in skilled nursing and transitional care. It is a five-star facility licensed by Medicare and Medicaid, recognized as a 2024 Best Nursing Home by U.S. News and World Report and is open to people of any faith tradition.

An RAI Coordinator is responsible for:



  • Schedule completion of RAI following state and federal guidelines.
  • Coordinate/supervise documentation/completion of MDS, RAPs and Care plans by the IDT Team.
  • Screen Minimum Data Set for accuracy before finalizing, validating, and signing.
  • Ensure full resident assessment, and when a significant change of condition occurs.
  • Ensure completion of comprehensive quarterly assessments are completed as scheduled.
  • Train other staff in completion of the MDS and corresponding documentation.


This position will be scheduled Monday - Friday - Full-time hours.

Qualifications:



  • Must have a current active license as an LPN/RN in the State of Ohio
  • Must have previous long-term care experience as a nurse.
  • Proficient in computer software/EMR documentation.


Special knowledge, skills, and abilities:



  • Demonstrated knowledge of State and Federal regulations, including Resident Assessment Instrument.
  • Demonstrated extensive knowledge of Case Mix.
  • Demonstrated extensive knowledge of Medicare, HMO's and Managed Care.
  • Good written and verbal communication skills.
  • Demonstrates knowledge in ICD9/10 Diagnosis Coding
  • Flexible with time management and prioritization skills.
  • Ability to maintain an effective working relationship with nursing staff and other departments.
  • Demonstrated knowledge of software (Micro Soft, ROX, Facility Software, Excel.)
  • Ability to work with others in identifying and improving quality of care issues and solutions.
  • High level of respect and commitment to working independently and with others. Has respect for others.
