Job Description
Job summary The Sr. Care Coordinator (CC) is a critical member of the clinical care team. He/she is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients enrolled in the chronic care management program. The Sr. Care Coordinator will be a subject matter expert (SME) in the care coordination department, performing all care coordination functions, and a resource to the care coordinators. Responsible to the clinical team and reports to the Care Coordination Manager.
Position Essential Functions: Job Responsibilities:
- Provide administrative support to the clinical care team
- Answers incoming phone calls for general inquiries and redirect as appropriate
- Route incoming faxes and assign as appropriate
- Assist with the implementation or finalization of patient orders that do not require an RN to complete
- Facilitate medical records management for outside documents that are critical for patient management, i.e. laboratory results, discharge summaries, medical documentation from specialists and primary care provider
- Support scheduling appointments and reminder calls or texts for chronic care management appointments
- Assist with referrals and referral management
- Assist with follow-up to assure needed DME is ordered and delivered
- Provide oversight for all Care Coordination functions including but not limited to orders, referrals, disenrollment, scheduling, error processing, medical records and consent uploading, mail retrieval and sorting, email processing, etc.
- Providing input to your manager regarding workload volume and staffing needs.
- Managing five9 and the daily assignment
- Interact with members/providers to identify member needs, clarify information, research issues, and provide solutions in a timely manner
- Uses procedures, policy manuals, knowledge-base and other reference materials to assist in answering various general inquiries and issues.
- Effectively addressing unprofessional or non-constructive behavior situations. Triages specific and complex inquiries to appropriate
- Support a variety of data entry functions
- Complete other duties that support the chronic care management program as assigned
- Solid customer service, organizational, interpersonal, and motivational skills.
- The ability to interface with all levels of staff and management.
- Basic understanding of Microsoft Word, Excel, and email. Prior use of Five9 preferred.
- Excellent oral/written communication and interpersonal skills.

- Exceptional verbal and written Communication skills
- Understanding of medical terminology
- Completion of a Medical Terminology course preferred
- Medical office experience preferred
- Customer Service experience
- Strong work history
- Working knowledge of computers and ability to manage data within an electronic system
- Five years of work experience preferred
- Bachelor's degree preferred.
- Recent experience in a healthcare setting and/or 3-5 years of experience working in a customer centered, production driven environment preferred
- Bi-lingual highly valued, as an additional skill.
- Monday through Friday 8am-5pm or as directed by Manager.
- Flexibility is required.
