We are currently seeking a dedicated and experienced Inpatient Heart Failure Nurse Navigator to join our team. In this vital role, you will help shape the future of heart failure care through education, advocacy, clinical expertise, and program development. Nurse Navigator Heart Failure

What You'll Do

  • Serve as a clinical expert, patient advocate, and educator for individuals with heart failure.
  • Coordinate the care of heart failure patients from admission through discharge and support seamless transitions across the continuum of care.
  • Collaborate with the Advanced Heart Failure (AHF) Medical Director, Heart Failure Program Coordinator, and nursing leadership to optimize outcomes and reduce readmissions.
  • Support development and implementation of heart failure clinical guidelines and performance improvement initiatives.
  • Deliver patient- and caregiver-focused education and facilitate team-based staff learning.
  • Ensure compliance with DNV and other regulatory guidelines to maintain program certification.
  • Serve as a liaison within the health system and in the community, representing the heart failure program at various levels.

What You Bring

Education:
Bachelors Degree in Nursing Required

Experience:
Minimum of 3 years experience in cardiology Required

Licensure & Certifications:
Active RN license in the State of Florida Required
Basic Life Support (BLS) Required

Preferred Skills & Qualifications:
Membership in professional/academic organizations
Basic proficiency with Microsoft Office Suite (Word, PowerPoint, Excel)
