Hospice Case Manager, RN | Full-Time .8 | Mankato, MN Territory. The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of clients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serve the nursing needs of patients, families and groups throughout the lifespan. Weekdays between 8a-4:30p. On-call rotation including both weekends, holidays, and weekdays. Pay: The targeted pay range for this job is $40.00 - $48.00 per hour.
