Job Summary:



The Healthcare Professional will be a part of a behavioral health team and multi-disciplinary clinic that specifically serves children with medical complexity. The patient age range is from infancy to young adulthood, and many patients also have a developmental disability and / or medical complexities. The Healthcare Professional role responsibilities will include collecting specific information from patients and families, working collaboratively with a multi-disciplinary team to advance the designated care plan and recommendations, facilitating connection between patients and families with professionals internal and external to the institution, communicating with behavioral health and related service agencies to facilitate referrals, and maintaining an organized resource repository. Additional responsibilities include working with patients, families, providers, and care teams to identify and address barriers to accessing services, resources, and supports that enhance the behavioral health and wellbeing of patients and families. The Healthcare Professional will enter data obtained from each distinct point of contact into the secure database management system and/or medical record. A goal of this position is to improve the coordination, integration, and access to behavioral health and related services for patient and families to ultimately enhance their behavioral health and wellbeing and reduce the care coordination burden experienced by caregivers.

Key Responsibilities:



  • Assist behavioral health team members or other professionals by interpreting and acting as a resource on rules, regulations, and policies related to patient navigation. Advise others on issues or matters and weigh in on discrepancies.
  • Responsible for coordinating the care plans, making certain all members of the team are communicated with and that the patients can be scheduled in a timely manner into our referred agencies/programs/community-based services.
  • Performs daily review of clinic schedules and miscellaneous clerical duties to insure proper patient flow in the clinic.
  • Create and maintain referral tracking reports to ensure patients are connecting to the necessary services.
  • Organizing and establishing work processes to provide support to management, ensure the delivery of designated services, and implement rules, regulations, policies, and other guidelines as prescribed by management to achieve program objectives.
  • Maintain and regularly update resource repositories and databases.

Onsite - this role is expected to work onsite and is located in Aurora, CO.

Qualifications:



Minimum Qualifications:

Entry Level:



  • A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.


Intermediate Level:



  • A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
  • One (1) year of professional level experience as a care coordinator, care navigator, or similar role.


A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:



  • Two (2) or more years of experience with case management, care coordination, or systems navigation.
  • Experience in healthcare setting or community-based organization.
  • Experience managing and handling sensitive and confidential information.
  • Experience working with students, staff, and faculty, within a higher education and/or academic medical environment with child & adolescent populations.
  • Experience working with EPIC or another electronic medical system.
  • Experience working with individuals with complex medical needs and/or developmental disabilities.
  • Bilingual or Multilingual - able to perform job responsibilities in English and another language(s).


Knowledge, Skills, and Abilities:



  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout the institution and in the community.
  • Knowledge of behavioral health/health care systems and/or organizations that support physical or behavioral health.
  • Strong interpersonal and patient care skills with the ability to establish rapport with and provide high quality care to persons of diverse backgrounds.
  • Strong problem-solving skills, and the ability to work independently with minimal supervision.
  • Collaborative approach to multi-disciplinary patient care and compassionate approach to patient and family care
  • Self-motivated, aware of own learning style and how to learn new tasks most efficiently,
  • Excellent time management and organizational skills with the ability to multitask.
  • Ability to take direction and use independent judgment to coordinate various projects and meet multiple deadlines.

Anticipated Pay Range:



The starting salary range (or hiring range) for this position has been established as:

Entry Level: $56,555 - $71,939

Intermediate Level: $61,546 - $78,286

The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
