Position Highlights |
The Division of Infectious Diseases in the Department of Medicine is seeking a dedicated and highly motivated self-starter to join the Petersen Clinics dynamic, multi-disciplinary HIV team as a Wellness Program Coordinator (Care Coordinator III). This position plays a critical role in supporting patients with complex medical and psychosocial needs by facilitating access to behavioral health services, financial and medical assistance, and stable housing resources across southern Arizona. Working closely with the Wellness Program Manager, the Wellness Program Coordinator will identify patient needs, anticipate barriers to care, and implement targeted interventions that promote engagement in HIV care and improve self-management within a medical case management framework. The role also includes assisting in the administration of the Emergency Financial Assistance (EFA) program, ensuring timely processing and accurate documentation of financial support services. This is an Extended Temporary Employment (ETE) position with the possibility of becoming a permanent position after two years should funding be secured. Duties & Responsibilities |
- Conducts initial and ongoing in-clinic assessments in accordance with clinical guidelines and federal and state funding requirements, developing and implementing individualized care plans that integrate life-saving medical services, social support, and treatment adherence strategies.
- Maintains strong communication with the multidisciplinary team and external partners to optimize outcomes, while educating and counseling patients on risk reduction, healthy behaviors, and other topics that address needs and enhance quality of life.
- Acts as a lead staff to support coworkers less equipped in behavioral health coordination and or crisis intervention.
- Administration of the Emergency Financial Assistance (EFA) program, which includes overseeing referral processing, coordinating the timely distribution of financial assistance, and managing the logistics of food gift card provision.
- Ensures accurate data management to support reporting, compliance, and program accountability, while connecting clients to essential community resources and housing wait lists, strengthening service delivery and client outcomes in consultation with the Medical Case Management (MCM) Program Manager.
- Manages accurate, compliant documentation of activities in designated databases, adhering to ethical guidelines, state regulations, and internal policies.
- Safeguards the integrity, usability, and confidentiality of data-especially Electronic Medical Records (EMR)-to support seamless patient care. EMR entries must be precise, as they are subject to review by internal and external clinical providers and funders for audits.
- Manages and coordinates a complex caseload of patients with dual chronic conditions, requiring advanced organizational skills to multitask and track needs-such as eligibility and social determinants of health-from intake through program discharge.
- Addresses multifaceted patient care challenges through evaluation, interpretation, and problem-solving, applying extensive knowledge of the Ryan White Program, behavioral health systems, local resources, and emergency financial assistance programs.
- Utilizes creativity and innovation in case management to secure funding, access resources, navigate the healthcare system, and resolve complex insurance issues.
- Reconciles data to ensure compliance with funding requirements for both the annual Ryan White Services Report (RSR) and monthly reports, enhancing care planning and supporting timely completion of acuity assessments in alignment with program standards under the guidance of the MCM Program Manager.

Minimum Qualifications |
- Bachelor's degree or equivalent advanced learning attained through professional level experience required.
- Minimum of five (5) years of relevant work experience, or equivalent combination of education and work experience.

Preferred Qualifications |
- HIV and behavioral health clinical coordination experience.
- Working experience utilizing local behavioral health services, financial and medical assistance, and stable housing resources in southern Arizona.
- Experience with Ryan White Programming.
- Master's degree in public health or social work or other related advanced degree
