Position Title: Resource Specialist
Work Location: Member-Facing - New Port Richey/Clearwater, FLorida Assignment Duration: 3 Months Contract to Hire Work Schedule: 8 am-5 pm, Monday through Friday Work Arrangement: Field-based / Remote (work from home and into the field) Position Summary: * This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member. Background & Context: * Compliance with care management policies/procedures including outreach and documentation expectations. * Individuals will be audited on a monthly basis to ensure charts are in compliance. * The first day will align with training/onboarding. The first day will consist of setting up computer, completing onboarding tasks, reviewing training schedule, and meeting with the training team. Onboarding is 4 weeks. Qualification & Experience: * Pediatric case management experience * Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc. * Bachelor's Degree * Microsoft Office- Outlook, Word, and Excel
