Position Title: Care Manager

Work Location: Remote (Alachua, Citrus, Lake, and Marion)

Assignment Duration: 3 months (Possibility to extend or convert?)

Work Schedule: 8am-5pm ET, no OT

Work Arrangement: Member-Facing

COORDINATING CARE FOR SPECIAL NEEDS FAMILY ON THE CMS PLAN

Key Responsibilities:

Set tasking of assigned caseload Max 200 members
Virtual with a couple of in person meetings per year

Qualification & Experience:

Required: Master's Degree
Required: LCSW/LMHC license active in Florida
Must haves: Pediatrics - Minimum of 2 years
Nice to haves: Bilingual Spanish/English preferred, Experience with electronic medical records, Basic Excel skills
Disqualifiers: Non licensure
Performance indicators: Caseload compliance
