Team Lead, Pathways to Success Care Coordination

Position Overview:

This position will provide clinical and administrative oversight to the Pathways to Success Care Coordinators. This full-time leadership role is responsible for overseeing the delivery of intensive care coordination services to Illinois Medicaid-enrolled youth under age 21 with complex behavioral health needs. The Team Lead will ensure fidelity to the evidence-informed Care Coordination and Support model, support staff development, and maintain high standards of service quality and compliance.

Knowledge, Skills and Abilities:

  1. Ability to work as a team player.
  2. Ability to tolerate ambiguity, uncertainty, and change.
  3. Ability to navigate electronic health records.
  4. Demonstrated above average computer skills, proficient in Microsoft Word and Excel.

Core Responsibilities:

  1. Provide clinical and administrative supervision to Pathways to Success Care Coordinators.
  2. Ensure adherence to the Care Coordination and Support model, including structured procedures for engagement, planning, and monitoring.
  3. Support staff in assembling and facilitating Child and Family Teams, ensuring inclusion of both formal and natural supports.
  4. Monitor service delivery across settings (home, community, schools, and agency) to ensure responsiveness to youth and family needs.
  5. Conduct regular team meetings, individual supervision, and performance evaluations.
  6. Provide training, coaching, and professional development opportunities to enhance staff competencies.
  7. Completion of all documentation and tracking in a timely manner
  8. Provide other duties as assigned, required or specifically negotiated with supervisor

Education and/or Licensure Requirements:

  1. Candidates are required to have designation as a Qualified Mental Health Professional
  2. A valid driver's license, reliable means of transportation, and proof of automobile insurance coverage are required.

Salary Range: $66,000-$70,000

Master's Degree in Human Service field: $64,000

LSW or LPC: $66,000

LCSW or LCPC: $70,000
