Description
The Clinical Case Manager: Program Manager is responsible for overseeing the planning, implementation, and evaluation of case management operations to support efficient patient flow, optimal Length of Stay (LOS), and reduction of avoidable days. This role ensures that care coordination processes meet quality, regulatory, and financial standards while promoting high-quality, patient-centered care. The Program Manager provides leadership, training, and operational oversight to the case management team and collaborates closely with interdisciplinary partners to improve throughput across the continuum of care.
Qualifications
Minimum Education Bachelor's Degree (Required) Or Master's Degree (Preferred)
Minimum Work Experience 5 years of related experience (Required)
Functional Accountabilities Program Management & Development
- Assist in development, implementation and evaluation of services, programs, policies and procedures, and performance standards to
- Assist in the development, evaluation and implementation of new ventures, services and partnerships.
- Create project plans with milestones, deliverables, and accountability assignments.
- Implement strategic initiatives through management of day to day program operations; troubleshoot operational issues and develop action plans to resolve.
- Collaborate on significant operational/issues/barriers.
Budget and Financial Management
- Prepare documentation and projections to support the operating and capital budget planning process.
- Evaluate and recommend purchase of new equipment needed to maximize productivity and quality of care.
- Implement actions to facilitate meeting budget requirements and administer programs within budgetary guidelines.
- Monitor monthly performance reports and budget variances and resolve discrepancies as needed.
- Evaluate reimbursement and denial trends and develop and implement action plans to address issues.
Business Acumen and Managing Results
- Understand key financial and budgetary principles and apply and incorporate them into management planning and decision making.
- Understand organization's business environment (e.g. regulatory bodies, strategic alliances, community partnerships, etc.).
- Develop and continually monitor budgets, fiscal performance and resource allocation.
