Sixteenth Street Community Health Centers is seeking a compassionate and resourceful Social Services Patient Navigator to support patients in accessing essential services and navigating complex life challenges. This role is ideal for someone passionate about community health, education, and care coordination.

JOB RESPONSIBILITIES:

  1. Patient Assessment & Coordination



  • Assess patient needs using Motivational Interviewing.
  • Develop service plans for patients with low-level and mid-level needs and schedule appointments for high-level cases.
  • Maintain accurate records of patient interactions, service plans, and outcomes in the Electronic Medical Record to ensure seamless care coordination.



  1. Education & Empowerment



  • Provide education and support to individuals and groups on various Social Determinants of Health (SDoH) including but not limited to safe housing, economic stability, access to community resources, and child safety.
  • Offer guidance on navigating healthcare systems and social service programs.
  • Support patients in understanding eligibility requirements for public benefits and community programs and assist in completing applications.



  1. Community Engagement



  • Research and maintain a database of community resources.
  • Build relationships with local organizations to support referrals and care coordination.
  • Serve as the point of contact for donations, gift giving, and supply drives for the Social Services Department.



  1. Team Collaboration



  • Assist with onboarding and oversight of volunteers/interns.
  • Participate in department meetings, trainings, and team initiatives.



  1. Backup Support



  • Serve as backup to the Social Services Intake Coordinator during absences.
  • Answer incoming patient calls and assist with scheduling.
  • Monitor and schedule referrals from the workqueue to ensure timely follow-up.



  1. Other Duties



  • Adhere to AIDET customer service standards and TB control policies.


  • Perform other duties as assigned.


QUALIFICATIONS:

  1. Associate degree in Human Services or 2+ years of experience in social services.
  2. Ability to obtain the following certifications within 12 months:

    1. Community Health Worker
    2. Child Passenger Safety Technician


  3. Preferred: Motivational Interviewing experience.
  4. Strong communication and MS Office skills.
  5. Ability to work effectively with diverse populations.
  6. Bilingual (Spanish/English) required.
