Overview

The External Partnerships Specialist serves as a primary liaison between the Carle Health organization and external clinical partners, supporting EpicCare Link and Community Connect initiatives. This role is responsible for coordinating onboarding and contracting for new partner sites; managing access requests; providing training, go-live support, and front-line troubleshooting; and ensuring compliance with HIPAA and organizational policies. Through regular rounding, education, and performance reporting, the specialist promotes system services, supports referral and continuity-of-care best practices, and strengthens relationships with skilled nursing facilities (SNFs), assisted living facilities (ALFs), and other community partners. The position also provides limited project management support and contributes to broader EHR-related activities that enhance partner engagement with Carle Health and better support our shared patient populations.

Responsibilities

Manage contracting of all new EpicCare Link sites (clinical, SNFs and ALFs) across all regionsSupport EpicCare Link access for the Carle system which includes reviewing access requests, collaborating with Compliance and Epic IT team, initiating required agreements, providing access to user training and executing on new functionality to appropriate users.Collaborates with ambulatory nursing and leadership, external providers, internal providers, IT-EHR, Business Development, Social Work, Radiology, and Health Information Management to support external partners.Lead, manage and/or establish Regional Work Group meetings across all regions. Support and facilitate the development of productive, collaborative, and value-added relationships between Carle providers and regional providers in the Carle service area.Provide regular rounding on EpicCare Link sites across all regions and Community Connect partner(s)Develops and maintains knowledge base of current Carle provider specialists and the breadth and depth of Carle specialty services.Provide front-line support for EpicCare Link issues and troubleshootingValidate HIPPA training has been completed for all new SNFs and ALFsProvide limited project management support for Community Connect partnersSupport ongoing reporting requirements for Community Connect partnersSupport other EHR activities that involve external partners or patient educationParticipates in new provider orientations, service line department meetings and travel to outreach locations as needed.

Compensation and Benefits

The compensation range for this position is $28.82per hour - $49.57per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
