This internship provides the opportunity to work closely with the Population Health team on initiatives designed to improve care delivery and health outcomes for patients with complex needs. The intern will gain handson experience supporting population health efforts that promote coordinated, patientcentered care across the system. Education High School Diploma or Equivalent required Knowledge, Skills and Abilities Must be reliable and punctual. Good time management skills. Strong oral communication skills.
