The Patient Navigator Coordinator is responsible for providing resources, education, referrals and coordination of care to patients. The Patient Navigator will develop relationships with CCNW patients and providers by providing resources, support programs and assist in meeting other cancer-related needs. Supports and adheres to the Cancer Care Northwest Compliance Program, including the Code of Ethics and Business Standards.
Responsibilities
KEY RESPONSIBILITES
- Provides CCNW cancer patients and families resources, support programs and services. Provides additional information about community resources for needs articulated by patients. Assists patients in navigating identified barriers to treatment by assessing needs and providing education to connect them with resources.
- Identifies gaps in resources and works within the community to bridge those gaps when possible. Serves as liaison between community organizations and patients to distribute donated items.
- Solves practical problems related to needs such as lodging and transportation during cancer treatments; identifies other barriers that would affect the patients' ability to participate in their treatment plan. Addresses emergent patient needs in the clinic as they arise, such as lodging and transportation needs.
- Conducts follow up calls to patients, when possible, to ensure that the resources provided met their needs and identify additional needs.
- Maintains accurate, confidential records documenting services provided.
- Works within the facility to promote navigation program and build patient referrals.
- Works cooperatively with the Billing department to improve patient processes.
- Assists patients in moving through the complexities of the organization. Coordinates with nurses, physicians, schedulers, and other staff to facilitate patient care. Responds to CCNW team requests in a timely manner.
- Attendance of team meetings to discuss patients and clinic workflow.
- Travel required between Cancer Care Northwest sites as needed.
- Performs other duties as assigned.
Qualifications
Minimum Requirements:
- High school diploma or GED
- 2 years of direct patient care experience
- Current CPR certificaton
- Previous experience working in a health care setting is preferred
- Previous experience with medical, radiation, and /or surgical oncology/hematology patient population preferred
CRITICAL SKILLS
- Strong organizational and interpersonal skills
- Strong knowledge and proficient use of an electronic medical record (EMR) system
- Proficiency in reviewing medical records and other patient care information
- Strong knowledge and understanding of HIPAA compliance and patient confidentiality regulations and policies
- Ability to prioritize and re-prioritize quickly
- Ability to develop collaborative relationships and work in teams, including medical, surgical, and radiation oncologists and referring providers
- Strong written and verbal communication skills, including medical terminology and therapeutic interventions available
- Ability to work with minimal supervision
ADDITIONAL KNOWLEDGE AND SKILLS
- Strong customer service skills
- Strong knowledge of oncology patient care and outpatient practice workflow
- Intermediate computer skills, including MS Office applications, internet querying, etc.
- Knowledge of local community resources
PHYSICAL ABILITIES To perform this job successfully, an individual must be able to:
- Regularly: sit, grasp, reach, bend, stoop/crouch, talk, and hear
- Occasionally: stand, walk, lift up to 30 pounds, pull/push up to 30 pounds, carry up to 30 pounds
- Vision: clearly see up to 5 feet with or without corrective lenses
WORKPLACE ENVIRONMENTAL CONDITIONS: While performing the essential duties/responsibilities of this job, the employee will not be exposed to any workplace environmental conditions caused by extreme noise, heat, cold, workplace hazards, atmospheric dusts/fumes/vapors/mists, pathogens, or toxic chemicals.
