The Clinical Health Advisor supports Medicare Annual Wellness Visit (AWV) programs and preventive care through protocol-driven clinical support activities performed under the direction of a licensed provider. This role does not include independent diagnosis, clinical decision-making, or treatment planning. In collaboration with providers and care teams, the Clinical Health Advisor ensures accurate documentation, patient engagement, and consistent execution of wellness workflows. This role is part of a centralized CHMG team and is not permanently assigned to one practice location.

Essential Job Function

Preventive Care & AWV Support

  • Identify patients eligible for Medicare Annual Wellness Visits and preventive services
  • Support AWV completion using templates and protocols for structured data collection, screening administration, and documentation
  • Review patient records to identify potential care gaps using standardized tools and protocols for provider review

Clinical Support Functions

  • Collect, document, and report patient health information and screening results to support provider evaluation
  • Perform protocol-driven preventive care activities in accordance with established workflows, standing orders, and provider direction
  • Perform and document screenings and health maintenance activities per established protocols
  • Administer medications and treatments as delegated by a licensed provider and in accordance with regulations and competency validation

Care Coordination & Patient Engagement

  • Communicate with patients to support visit preparation, scheduling, and follow-up
  • Provide standardized patient education using approved materials and reinforce provider instructions
  • Assist with implementation and reinforcement of provider-directed care plans
  • Report patient concerns, abnormal findings, and follow-up needs to the provider or appropriate care team member

Documentation & Workflow Support

  • Accurately document patient information in the electronic medical record
  • Support workflow consistency across practices using standardized processes
  • Assist with referrals, scheduling, and patient communication tasks

Collaboration & Program Support

  • Collaborate with providers, care teams, and centralized program staff
  • Participate in process improvement initiatives to support wellness outcomes
  • Adapt to multiple practice settings while maintaining standardized workflows

Travel Requirement:

  • This role is part of a centralized CHMG team and is not permanently assigned to one practice location
  • Team members are expected to travel to different CHMG primary care practices to meet business needs, coverage needs, practice-specific workflow demand, and performance goals.
  • Practice assignments will be evaluated monthly and may change frequently based on operational needs, AWV performance, staffing support needs, and program priorities.
  • Employees in this role must be able to adapt to different primary care practice settings while maintaining standardized AWV workflows and expectations.

Education

Required: High School diploma or equivalent

Preferred: Associate degree in Health Sciences / Medical Assisting or Practical Nursing

Experience

Required: minimum 3 years of related experience in primary care, ambulatory care, preventive care, care coordination, or medical office clinical support. An associate degree in medical assistance or related healthcare field may be substituted for one (1) year of required experience.

Participation in a clinical orientation, competency evaluation, and ability to pass a clinical skills assessment and medication administration test within ninety (90) days of hire, and within 3 attempts.

Licensure/Certification/Listing

Required - One of the following:

* Current, unrestricted Licensed Practical Nurse (LPN) license

* Certification as a Medical Assistant (CMA or RMA) from one of the following organizations:

- American Association of Medical Assistants (AAMA)

- American Medical Technologists (AMT)

- National Center for Competency Testing (NCCT)

- American Allied Health (AAH)

- National Healthcareer Association (NHA)

- American Registry of Medical Assistants (ARMA)

- National Healthcare Workers Association (NHCWA)

- National Association for Health Professionals (NAHP)

- National Registry of Medical Assistants (NRMA)

- American Medical Certification Association (AMCA)
