The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Position Summary

The Clinical Coordinator II RN in the Division of IBD Surgery will be responsible for the initiation of the patient's history and physical, development and implementation of a pre- and post-office visit and procedure education program. The incumbent will support clinical functions of the IBD Surgery Service to provide compassionate patient-centered care and ongoing quality improvement within the division. The candidate will be responsible for the case management of patients including coordination of evaluation of diagnostic testing, reviewing lab reports, and supervising direct patient care. In addition, s/he will also facilitate coordination of home care services and patient education.

Responsibilities



  • Assess health status by completing a nursing assessment.
  • Performs telephone triage for (2) physicians.
  • Notify physician/LIP of abnormal findings.
  • Establish a care plan in collaboration with the patient, family, and health care team.
  • Collaborate with other care coordinators to ensure smooth and effective transitions of care.
  • Assist with specialized diagnostic and therapeutic procedures.
  • Assist with specialized patient care equipment as required by the clinical department and medical specialty assigned.
  • Administer medications and treatments (including infusions) as ordered by physician/LIP and monitors patients for response.
  • Initiate CPR and other emergency measures. Coves other nursing roles within the department
  • Provide health education to patients and families. Develop, evaluate, adapt, and document education to learning needs. Assist in preparation of the department specific education for patients and colleagues.
  • Facilitates smooth operation of the ambulatory areas. smooth discharge of patients from Outpatient Endoscopy area as needed.
  • Coordinate care for IBD and colorectal patients including patients from out of state to streamline office visits, testing, imaging, endoscopy and surgery.
  • Participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience, and patient safety.
  • Maintain professional growth and development in the nursing field and identifies annual core competencies.
  • Other duties as assigned.


Minimum Qualifications



  • Bachelor's degree or equivalent combination of education and 4 yrs work experience required.
  • New York State RN license required.
  • BCLS
  • Intermediate MS Office skills


Other Requirements



  • Ability to perform comprehensive history and physical examination.
  • Ability to manage and coordinate a patient caseload.
  • Work within a collaborative agreement/protocols with a physician and health care team.
  • Demonstrate leadership ability to manage patients and interact effectively with peers and physicians.
