Works with patients who present with AODA and/or Mental Health concerns. Completes psychosocial assessments, treatment plans, and ongoing treatment with individuals, groups, and families in cooperation with patient and multidisciplinary team. Consults and coordinates care of patient with physician, providers, and community agencies. Delivers behavioral health care in accordance with patient evaluation, treatment plan, and within the scope of their license. Maintains pertinent patient data in medical record in accordance with state, Joint Commission, and Mercy Health System guidelines. Performs other duties as assigned. Complies with all Mercy Health System, State, and Joint Commission policies and regulations. Must be willing to travel between treatment sites, on occasion, when asked to do so during situations where program coverage is required, meetings, and/or system in-services are required. Culture of Excellence Behavior Expectations To perform the job successfully, an individual should demonstrate the following behavior expectations: Education and Experience Master's Degree in Counseling, Social Work, Psychology or other related field. Certification/Licensure Licensed to independently provide both mental health and substance abuse treatment in Wisconsin.
